Repeat prescriptions

You can request a repeat prescription in a number of ways, if you cannot pick up the prescription please send us a stamped addressed envelope (SAE):

1. By completing the form below, please allow 2 working days notice and you can pick up your prescription from the surgery. Periodically you will need to see a Doctor to review your medication.

2. Prescription requests can also be sent via e-mail to
wc-pct.repeatprescriptionsohs@nhs.net
If using this method you must provide the full name and doseage of all medications you require. Ask at reception if you would like a copy of the list of repeat medications we have for you.

3. You can put a request in the request box inside the front door of the surgery

4. Fax us on 0151 356 2683

Online prescription Form

Please note the following informaton which is to assist you in using this on-line form for repeat prescriptions

  • This form must not be used for anything other than repeat prescriptions
  • The practice will not reply to this on-line form
  • The forms will be checked once a day in the morning and prescriptions will be ready 48 hours later
  • This will be in our working week of Monday to Friday
  • Please ensure that you allow 48 working hours for repeat medication
  • If you request medication that is not on repeat then a message will be left at reception for you
    • If need a list for repeat medications please ask the doctor or receptionist
  • Please ensure that you use the name of the medication from the repeat list. DO NOT USE THE NAMES FROM THE BOTTLES/PACKS
