Registering

We welcome patients from within the Practice area, which covers Ellesmere Port, Stoak, Whitby, Little Sutton, Overpool, Backford Cross and Wolverham.

To register you will need to fill in a registration form (download the PDF here or call in to get a copy). The person requesting to register must bring this form to the surgery to register.  We will ask you to provide proof of your address e.g. utility bill or driving licence

An appointment will be made for you for a New Patient Check and you will be asked to bring a urine sample with you.

If you are on any regular medication you will need to see the Doctor to have this made available as a repeat prescription.

The Practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.

Change of personal details

If you move or change your name or telephone number please help us by informing us so your records can be updated. Failure to do so could cause delays or prevent us from contacting you or a relative in an emergency.

Medical records / Electronic Health Records

All patient records are recorded on computer and some data is held in paper files. We comply with the relevant Data Protection Act and all staff are trained with regard to their responsibilities.

The Electronic Health Record is a new system for storing information about your health. It will enable NHS staff from any hospital or surgery to retreive the latest data on your Health in the event of an emergency. For more details please pick up a leaflet from the surgery.

Freedom of Information Act

All Health bodies are covered by the Freedom of Information Act which gives all interested parties a general right of access to all types of recorded information.

We have a publication scheme that can accessed at www.foi.nhs.uk. If you have any questions please ask the Practice Manager.
